Yeah, I've been lying low for some time mainly because I had work, but I made an agreement with a neogaf member to make a printable version of this box. Unfortunately I think this is 200 dpi or something so I don't know if this will work or not.
Anyway, credit to Sens for the dualshock logo and the temp which I modified for a US version (hope you don't mind, mate).
Rest is usual.

Its not excatly 300 dpi, so I'm not sure whether or not you might have to upscale it or not.
