    NOTE: I've never 3D'd a box before so this will be extremely crappy. Also, I couldn't do the spine. You'll have to add that in yourself.

    Try using this as a guide. Separate the front, spine, and back into three sections and skew them appropriately. You really only need to skew the back in this view, the other sections are straight for the most part.

    Had some free time on my hands and was bored. heres one with a reflection and without case. The quality you gave us is reallyyyy low so did the best I could with what you gave me.

    EDIT: and yea you guys should check out the tutorial...

    Looks great. I'd add an alpha channel gradient on that reflection. It usually makes it look a little nicer.
