(Unfortunately since I use Imandix it doesnt let me export the box rendered so I tried my best for the presentation)
And Imandix cant render the box type you were looking for, I tried my best to replica but to be honest if I were you I would ask someone who has experience with skewing boxes to 3d in photoshop.

Thanks for the 3D. Could you also send me the version without the template? Like, just the 3D of my box by itself?
